THE NEED TO BE GRATEFUL FOR MOST ABORTIONS

She was the mother of two children, radiant and full of vigor.
Her life was normal until she took in the third time. Little did she know that pregnancy would consume her.
She presented to the hospital with history of loss of menstruation, lower abdominal and back pain, weight loss and intractable bleeding per vagina. Before her presentation she had been in a peripheral hospital and had received about 10 units of blood.
The test for pregnancy was positive but the predictive value was alarmingly elevated. Other investigations done supported the diagnosis of a molar pregnancy which had over time transformed into cancer. The chemotherapy wasn't making her condition any better, pain was her constant companion, she bled more than the woman with the issue of blood. She was constantly transfused with blood and all their resources drained. Her husband became frustrated and requested for the uterus (womb) to be removed but no, it was late, the cancer had spread to the bladder, rectum and backbone. Everyone had been exhausted including the health care providers. She could no longer fight to live, her end had come, in tears she gave one last look to her younger sister who had been by her bedside all along and to her weary mother who had been praying and earnestly waiting for a miracle, then to her dear husband, who held tightly to her hands as if in a tug of war game with death. She apposed her eyelids gracefully and passed on with tears rolling down her cheeks.
ABORTION

It stings like a bee, hurts like a sore, painful like labour and agonising like the sorrowful mysteries to have waited for years, spending all the energies in sex, with great expectations for conception which inadvertently ends in abortion. But would you rather prefer it ends like the story above?

Before I proceed it's pertinent that we understand the concept of the term, abortion. It is the termination of pregnancy before the age of viability (when the baby can survive outside the womb). In advanced countries the age of viability is between 20-24 weeks while in less developed countries it's about 28 weeks. Nevertheless, when this termination is initiated by human intervention using any means and not based on medical reason (s) , it's termed 'Criminal Induced Abortion'. On the contrary, when it occurs by itself, it's termed 'Spontaneous Abortion' and this is where my interest lies.

CAUSES OF SPONTANEOUS ABORTION


  • Chromosomal/ Genetic abnormalities 
  • Maternal Disease conditions ; Diabetes, Renal disease. 
  • Infections 
  • Genital tract anomalies
  • Cigarette smoking 
  • Alcohol 
  • Toxins including certain drugs 
  • Radiation
  • Unknown
ESSENCE OF GRATITUDE 

There are two main reasons for gratitude to God for some cases of spontaneous abortion. 

  1. Aversion of malformed babies
  2. Aversion of malignant cancer
Aversion of malformed babies 

Incidentally, most causes of spontaneous abortion are responsible for congenital malformation (abnormal structure at birth). The body has a surveillance system that screens, identifies and repairs /eliminates abnormalities at cellular, tissue, organ or systemic levels. This is done to terminate conditions incompatible with life and further prevent complications and preserve an individual. This regulatory mechanisms fail sometime at the initial stage and the child is born with such abnormalities but eventually dies sooner after birth or lives a life without quality. Consider the trauma posed by this situation on both parents as well as relatives. 

Aversion of malignant cancer ( Gestational Trophoblastic Neoplasia)


It is scary to know that pregnancy can transform into cancer. This is usually the product of chromosomal abnormalities following the fertilisation of an empty or normal ovum (egg) by 2 Y chromosomes instead of one.
When the surveillance system fails to terminate this abnormal (molar) pregnancy, it developes into an invasive mole, the cells become immortal and may transform to malignant cancer, Choriocarcinoma. It eventually spreads to near and distant tissues and results in death of the individual.

I'm yet to see a patient that survived this but it's not altogether a hopeless situation.

UNRAVELLING THE PUZZLE OF SYPHILIS IN A THREE YEAR OLD

... After Dr. Okpokiri announced the result of the VDRL test (test for syphilis) to the disappointment of Mrs. Oruada, neither of them was positive, he husband turned and gave her an angry look. The fire in his eyes could raze a sky scraper and she saw hell through his eyes that sent chills down her spine and reawakened her conscience to all her accusations and show of distrust for him. She quickly remembered the occasion where she regretted by words and gestures ever knowing her husband and pushing him down along the stairs when he tried to resolve issues with her. All the ugly incidents and her misbehaviour came back to her like the prodigal son and pierced her soul like the silver arrow in Robin hood, emotionally overwhelmed, tears flowed down her prominent cheeks while she cried like an infant hungry for breast milk. She knelt down, buried her head between her husband's laps while resting her elbows on the later and asking for forgiveness from him. Mr. Oruada was moved with pity that he clasped his arm around her neck and wiping her tears with his brown handkerchief and reassuring her of his forgiveness and endless love.

When this sensational episode was over, Mr. Oruada then asked the doctor, how then their child got syphilis if neither of them was responsible. They were adviced to take their time and tactfully inquire from their child or mount a surveillance since unthinkable things happen these days. That afternoon , the parents called little Daniel into their bedroom, gave him a cup of ice cream, the mother called him several pet names, stroke his hair, all in preparation to dig out the secret in him. When they they felt the time was ripe, she now threw the question at him, Who has been touching you here? While indicating his private part with her index finger. Daniel replied immediately, ' Is it not Ivie'. Shocked at his response she was filled with outrage. what? She asked furiously with an impulse that set her in motion to unleash the dragon on Ivie but her husband signalled with his hand asking her tobe calm. They then further asked him to narrate everything she had done to him.

Ivie was their house help, about 16 years of age. She slept in the same room (children's room) with the kids. At midnight, she would carry Daniel in her laps, stroke his penis and when well erect would introduce it into herself. Daniel got used to it that most times he would lie on Ivie and do certain other absurd things which she thought him. This had gone unnoticed by the parents for 7 months before he fell sick. Ivie outrightly denied all allegations levelled against her. She was taken to the hospital to be tested and behold the result was positive for Syphilis. The other story of what became of Ivie is better imagined than told.


SYPHILIS


This is a sexual transmitted disease caused by a gram negative organism called treponema palladium. It can also be transmitted through the placenta from mother to developing fetus during pregnancy.

SIGNS AND SYMPTOMS

The disease if untreated will progress through 3 stages, viz:


  • Primary Syphilis 
  • Secondary "
  • Tertiary     " 
Primary Syphilis is characterised by:
  1. A unique painless rash, Chancre on the external genitalia
  2. Painless solitary lymph node enlargement 
Secondary Syphilis is characterised by:
  1. Bacteremia with associated fever
  2. Condyloma latum
  3. Generalised lymphadenopathy 
  4. Systemic symptoms 
  5. Patchy baldness
  6. Generalised body rash
Tertiary Syphilis is characterised by:
  1. Gummatous lesions affecting the skin and bones 
  2. Cardiovascular Syphilis with aortitis (inflammation of the aorta), aortic aneurysm (ballooning of the aorta) and myocarditis (inflammation of heart muscles) 
  3. Neurosyphilis with associated General pares is of the insane, visual disturbance, subacute meningitis but may also be asymptomatic (without symptom). 

Effects of Syphilis on the Fetus

  • Spontaneous Abortion /Miscarriage 
  • Prematurity
  • Intrauterine Fetal demise 
  • Early infant death

PUZZLE OF SYPHILIS IN A THREE YEAR OLD

Common things are common and vice versa . This slogan is an aid to making diagnoses in medicine but the challenge is when uncommon things dare your medical prowess and sometimes leave you more confused than the patient. This was so in Daniel's case.

To be super-parents we need the eyes of an eagle, the team work of the 300 Spartans, the gentleness of the lamb and the wisdom of the serpent. Daniel is three years old and was brought to the clinic on account of recurrent fever and malaise. After a round of clerking the physician made a diagnosis of Poorly treated Malaria and commenced him on a more appropriate antimalaria medication.

After a week Daniel was brought back to the clinic with worse symptoms of generalised body rash and lympadenopathy (lymph node swelling) in addition to the earlier symptoms. At this juncture , the physician's tentacles were raised, at first he thought of measles but the more he dug into it, the more uncertain it became, he also thought of a possible HIV infection and lymphoma . On inspection of the patient, he found a wart-like growth on Daniel's scrotum which got him confused the more. He stood still for a while pondering over his findings, looking for a clue but his brain was blank and the mother kept asking him what the problem was with her son.

The patient was admitted and some investigations sent while palliative treatment was commenced but when the results were retrieved it wasn't any of the diagnosis he had in mind. Then he turned to the mother and asked her the million dollar question, the blow that broke the back of the camel, 'Madam, was there anytime within the past six weeks that your child had a rash on his penis'? Looking into his eyes as though he was a prophet with hope and confidence restored in her she gave him a nod and added that it didn't disturb her child but eventually resolved spontaneously with hich made her attach zero importance to it . He quickly pulled a leaf of investigation sheet lying like a comatosed patient and filled for VDRL test. The result came out positive but it did not go down well with Mrs. Gladys Oruada after the doctor explained the result to her and requested that she and her husband be tested as well.

It was the battle of Muzanga that evening when Mr. Uche Oruada returned from work, he received a cold welcome from his wife who was filled to the brim with suspicion and waiting in the bedroom to kick start the Amagedom. His confusion was interpreted as pretence. She unleashed hell on him that night, calling him a cheat, a womaniser, a wolf in sheep skin, a syphilis distributor, blablabla.... Mr. Uche had never seen his wife in this mood in the past eight years of their marriage, he was so disappointed but he managed to get a grip on his temper less he beat her blue black. The matter was laid to rest temporarily when the couple agreed to run the test but she refused to spend the night with her husband in her bedroom.

That night Mrs. Gladys after two hours of sleep walked up the stairs leading to her bedroom. She could hear loud moaning from that direction and as she drew closer the noise became louder. She opened the door and caught her husband ramming their house help, Ivie, from behind. Filled with rage, she picked up a wine bottle and flung it at them hoping to fracture their brainless skulls. The cracking sound woke her up and she realised it was just a dream and that she had smashed the flower vase in the sitting room against an aquarium.

No breakfast was served the next morning, both of them wearing long faces, no one uttered a word to the other, they just went their separate ways to the clinic to have the test done.
Life was no longer at ease for Mr. & Mrs. Oruada, their once peaceful home has become a theatre of war. Her husband started keeping late nights, the children, unhappy and miserable and the wife filled with bitterness and recked.
The doctor called the couple when the result was ready and both went with great expectations and anxiety . Mrs. Gladys was convinced within her that her husband would test positive but on the contrary, he didn't, neither was Mrs. Gladys positive to the test.

Roseline was the third daughter of Mr. Chukwuka, a man who all his life has been humiliated by the monster, poverty. His sweat stinks of suffering, his eyes sunken like a severely dehydrated child, the palms of his hands as hard as the petrous bone and as coarse as an emery sandpaper. A handshake with him gives his occupation away. Pray not to engage him in a fight for his hand is a deadly natural weapon. His wife will explain better. She gives him a considerable interpersonal distance whenever there is a quarrel and strategically positions herself beside the exit door with her left hand clasping the nob of the metal door opener for an easy escape should her husband make a move to touch her with his steel hand, while she runs her mouth like a tap.
Ijeoma, Chukwuka's wife, had known suffering all her life, moving from a poor background where her parents could hardly afford a square meal into the hands of her husband whose life have been recked by poverty . Her fair complexion had been concealed by the scourging sun to which she is constantly exposed while toiling to augment her husband's daily income. She couldn't join the women's wing in her church owing to the huge financial obligation and show of affluence which is common among the group. Her rivals nicknamed her Mrs. Job and when they're in their unholy company one would call out to her while another would reply, 'curse God and die' but like Mary, she treasured all insults in her heart. Despite the prevailing condition she raised her daughters in modesty and gave them formal education.
Roseline was beautiful, sexy, intelligent and innocent in her thoughts. She graciously completed her tertiary education as a biochemist from the Abia State University as the second best graduating student in her set. She was amongst the first batch of graduands to undergo the one year compulsory National Youth Service Corps Program (NYSC). She was posted to Lagos, Nigeria for the exercise.
Having obtained her first degree, she thought it was time to get married and wished she would find her Mr. Right in the orientation camp as it was common notion  among youth corpers. It was as though heaven answered her prayers when Stanley Ogbaegbe came her way. It was as if she had met an angel. He was nice to her and she loved him with her body, soul and heart as he was her first love. They hung out together, did everything in common. It was all gingles in her brain the very day Stanley proposed to her. She was so emotional that she allowed her vaginity to be stripped off her that same evening (was there any reason for her to withhold her body after the proposal?).
As days went by, she became more certain of spending her whole life with her lover, in fact, she couldn't wait for the service to be over. When it was time to fill personal data for The Certificate of Service, she threw away her surname and filled in Mrs. ROSELINE OGBAEGBE.
The service year had ended, Roseline and Stanley did not see each other at the passing out parade ground. She was actively involved in the parade but missed most of the marching steps and moves while trying to spot her lover in the crowd. Midway in the activity she collapsed and was rushed to the hospital. She was confirmed pregnant. Uncertain how Stanley would react to this, she placed a call across to him but he kept turning the calls down. She eventually sent a text message across to him but his response was a shocker.
He has probably changed his phone number to prevent her from getting access to him. She has lost her dignity, her pride, her menstrual cycle, her certificate, her first love and is about to become a single parent . Tears rolled down her cheeks as she sat in the bus conveying her back to her home town. She wished she had never met Stanley but more so, that she had not by her attitude ejaculated prematurely.

ROMANCE WITH DEATH


The darkest cloud. Something more bitter than vinegar, an amargo. An amalgamation of eclipse of the sun and moon. A tale of agony and anguish. The tears in his eyes weren't those of joy but those of a devastated man, a man whose essence of existence has been stolen. His life has become meaningless, so meaningless that Job's life became a luxury compared to his. The margins of his eyelids could not hold back his tears. His blood shoot eyes like crimson, traumatised by the tears, his nostrils in sympathy oozing out uncontrollable watery mucus. His heart beating in staccato and his weak and weary body left at the mercy of his comforters. He tried so much but failed to restrain himself from crying. His voice sounded like a cracked or broken record as he CRIED 'JULLIEEET IS GONE'

She was the apple of his eyes. His one and only love, his constant companion and advicer. They were compatible partners in crime,doing everything in harmony. Everyday Mr. Clement would thank God for his wife for she was a perfect soul mate. They  had been married for seven years and wished they would live together forever until this morning when the powerful cold hands of death snatched her away from her heartbroken and helpless husband.
Juliet had her first child in 2013 after which she endured secondary  infertility for three years. With the help of medical expertise she conceived a duplet which was delivered successfully. During her pregnancy, she developed Hypertension and was subsequently discharged home with some medications including antihypertensives after effective control of her blood pressure, postpartum (after delivery). She was also given a week appointment for checkup but both prescription and visit to the clinic was not adhered to. She probably thought all was well until the SILENT KILLER creeped in on her and closed the chapter of her life history, leaving behind her twin newborns and confused husband.
She was up almost through the night with the nursing experience, attending to both babies ;breastfeeding, changing diapers, trying to appease them equally as jealousy was expressed through crying from the one unattended to. All these activities went on while the August visitor was romancing her until about 5am when the symptoms of his presence were felt ; severe headache, palpitations and difficulty in breathing. She took her medication immediately but slumped 30 minutes later and that was it. She was rushed to the hospital with a pulseless, lifeless body but JULIET IS GONE!


GESTATIONAL HYPERTENSION (PREGNANCY INDUCED HYPERTENSION


Let me start by saying that HYPERTENSIVE DISEASES IN PREGNANCY is no joker. It's one of the four major KILLERS of pregnant mothers. This condition in which a non hypertensive pregnant woman eventually developes hypertension in the second half of pregnancy MUST AS A MATTER OF NECESSITY BE MANAGED BY AN OBSTETRICIAN and every instruction strictly adhered to.
PIH usually resolves within two weeks postpartum but can predispose the patient to essential hypertension, hence the need for regular medical checkup.

SLAPPED BY A GHOST!


It was a cold night, the cloud was as dark as Diana and had assembled over us to graciously bless us with rain. I had finished two wraps of fufu served with vegetable soup specially prepared by my beautiful wife. Such meals are usually suspicious as a request follows the delicacy at the end of the meal. Here she comes with the usual preamble, calling me pet names, sat on my laps, kissed me deeply and began to unbutton my shirt while curdling my hair. Suddenly there was a bang on the door with a feminine voice and a tone characterised by tension and confusion. I realised I was dreaming and jumped out of the bed as if I was pricked with a needle,my heart racing to escape a perceived danger while attempting to leave my sluggish body behind. I held my chest as though I was reciting my national anthem and as though to prevent my heart from leaving my body. It was the nurse on evening duty. She had come to announce an emergency case which had presented to the hospital. What's the case summary? I asked. I don't understand, she replied while scratching her hair and body. What don't you understand? I asked again. She stammered and answered, I heard he was slapped by a ghost! My thought drifted away momentarily as though a wave had blown across it to Umuahia, the capital city of Abia where sometime ago it was widely reported that ghosts slapped people to death in a popular brothel and not long after that a rumor went round that a sitting governor was also slapped by a ghost. My bizarre thoughts were aborted by a call from the nurse, the calibre of the voice this time, masculine. I looked at her just to see that her focus was below my belt, at a prominent figure which had responded to my romantic dream. I cleared my throat and urged her to go ahead while I pick some instruments. It took some effort to relax myself before proceeding to the emergency unit, to avoid further embarrassment. On getting to the emergency room, an elderly man with a unilateral dysmorphic face (hahaha, no offence, it means half part of his face was disfigured) as seen in the above picture, with drooling of saliva and dysphonia, altered consciousness and unable to raise the limbs of the affected side. His blood pressure was 220/150 mmHg with a high volume pulse.
His breakfast is a packet of cigarettes which he enjoys with whisky. He would go out of his way to make them readily available including borrowing. He owed Mr. Amanaha the sum of #5000 before his demise following a fatal motor accident. Adindu went outside the house to urinate at about 9.30pm that same night when he fell to the ground with above symptoms. They therefore concluded the late Mr. Amanaha attacked him on account of his indebtedness.

When blood fails to get to some parts of the brain either due to blockage by a clot (thrombus) or due to rupture of fragile blood vessel (s) (hemorrhage), the nerves fail to function. This is known as stroke.

 The common signs and symptoms are contained in the history.
Prognosis
  • 1/5 of affected persons die from it
  • 4/5 of affected persons survive but 1/2 of these suffer physical disability 
Endeavor to check your blood pressure regularly if you are 40years and above.

THE WEEPING BREAST IN AGONY OF INFERTILITY

TThe sight of her breasts could stimulate the involuntary movement of the head and make you change your direction to go her way. Conspicuous enough to defy the bounding effect of the best quality bra. It would pop out voluntarily causing her owner episodes of focal embarrassment, countless adjustments and unwarranted stop-to-woo by men with provoked and uncontrollable libido. The sight of her nipples would further provoke you and make you pledge your will to to have a taste of them. You can't but say hello to Ifeoma without your eyes focused on her chest as though you were expecting the response from them. She had three lovers before she eventually settled down with Benjamin. Ifeoma was concerned about her looks so she skips meals and endulges in strainous exercises to keep shape. These made her develop Chronic Peptic Ulcer Disease and and was placed on Cimetidine. Her life had gradually turned into a nightmare especially now that she had been married for three years and yet to conceive. Her husband is tired of fondling with the breasts since all attempts amounted to unproductivity, in fact the breasts have become objects of ridicule for her, more so now that she is having an uncontrollable spontaneous breastmilk discharge.


Her asset has now become a trauma  source. She is traumatised psychologically and presents to the clinic for help.

HYPERPROLACTINEMIA

Prolactin is a hormone produced by the brain (Anterior Pituitary gland) and reaches its target organs especially the breast through blood. It's normal concentration in the blood is about 8ng/ml. When it occurs in higher concentrations, it's termed hyperprolactinemia and can be caused by normal activities such as Pregnancy, Stress (psychological), Exercise, Nipple stimulation, Sleep and Estrogen (as found in contraceptive pills). Drugs like Cimetidine, Methlydooa, Phenothiazines and Tricyclic antidepressants may result to it.
Polycystic Ovarian Cyst and Tumor of the Anterior Pituitary gland are pathological conditions that can result in hyperprolactinemia.

Functions 
  • Synthesis - steroids and lactation (breastmilk formation) 
  • Development - breast and fetal lung
  • Metabolic - carbohydrates, Calcium, vitamin D and subcutaneous fat
  • Regulatory - inhibits the hypothalamus 

It's role in infertility 


It inhibits the hypothalamus from producing hormones ( Gonadotropin Releasing hormone) through which it instructs the pituitary gland to inturn release hormones that will cause egg development in the ovary and subsequently ovulation. Therefore with this link broken due to hyperprolactinemia , the eggs (ova) will not develop and ovulation as well as menstruation may not occur. Please visit your physician for care when your breasts weep in agony of infertility.

After laying down the foundation beginning with the concealer to make less apparent the blemishes on her round face, she coated her spindle shaped brown lips with a maroon red sleek lipstick and stood half way up, taking a closer look at the mirror, smiled in admiration to her artistic masterpiece, took some steps backwards as though she was practicing ballet, just to get a wider view of her shape. She placed her hands about her tiny waist and shook her buttocks aimlessly to give her the erroneous impression of natural resourcefulness.
She took her car key and walked majestically like the queen of England without a guard, swinging her waist like a tadpole from side to side. The gate was unmanned so she spent her little strength rolling it back and forth while she missed her husband who had been away for a business trip or perhaps....? Having completed this hell of a task, she sat in the car for few minutes trying to catch her breath with her chest rising and falling like the waves of the cardiac monitor. She took a second look at the inner car mirror, pulled out a dazzling handkerchief from her Gucci handbag and gently doused her face while whispering to herself, Jaaane! You are too lazy. She started the Highlander Jeep and drove off to a friend's house for a birthday party.
About three hundred metres from her house a little boy ran into the road from the roadside and reflexely she applied the brakes abruptly, dashed out of the car in response to adrenaline surge to probably beat the demon out of the boy but her mission was aborted when two men with the looks of The Undertaker and John Cena creeped out of the side bush, pushed her away with little effort and made away with the car.
She picked up her wig which fell off her head, jumped up and down and raised an alarm with her tiny voice but was helpless. She reported the incidence to the Nigerian police force in Onitsha Anambra, who assured her of their assistance.
One week later, some arrests were said to have been made with recovery of the vehicle. Jane never set her eyes on the recovered car rather she was shown a picture of the car, damaged beyond repair. She was allegedly offered #1. 3 million naira by the culprit through the officers of the Nigerian police which she refused initially. Some how the money eventually found its way into the cold hands of Jane. She also alleged that three hundred thousand was collected from her as logistics for the recovery, also that the culprits were freed after paying the sum of seven hundred thousand naira.
About one year now in the busy city of Aba, Nigeria, a young man visited the office of the Motor Licencing Agency to renew the papers of a Highlinder Jeep bearing the name, Mr. Okechukwu and included in the documents was a change of ownership with the original owner as Mrs. Jane Namdi. When the vehicle number was keyed into the computer, all details including phone number of Jane appeared. She was contacted immediately and she gave this story but before the agent could drop her call, the young man disappeared. The matter was reported to the police this time in Aba. The Agent and some family members of the young man are in police custody, the police division in Anambra has been notified also and the police men who handle the issue are also under custody but the young man who came to renew the vehicle licence is still at large.

MOTHER BLAMES EKAITTE FOR KILLING SON WITH SEX


There are three basic Sweet things in life, all controlled by the same gland, the adrenal gland and begin with the letter S; Sex, Salt and Sugar. Jeffrey loves life and always live it to the fullest and his wife was a perfect compliment, always ready for funfair. In fact this was the principal reason why he married her but Agatha, Jeffrey's mother wasn't in the least comfortable with her daughter-in-law. She perceived her as wild wind that blows no one any good but her husband never bath an eyelid over his mother's idea about his yuppie wife. He was well to do and so never hesitated to spoil his fun loving wife the more. One could scarcely find them in their home especially on weekends. Their cravings for fun informed their choice of contraception for five years after marriage. His life was work to fun. He considered himself strong enough and had no reason to attend the clinic for medical check ups, Ekaitte, his wife did say that two weeks before his demise, he had complained on two occasions of a nagging headache which he swept under the carpet with analgesic.

On this faithful day, he came back from a trip into the waiting arms of his dear wife. It was as though he had been away for centuries. They primed themselves with alcoholic aphrodisiac and off they went into ecstasy, moving from one planet to another as they rocked each other back and forth with responsorial psalms . On the third round of the clash of Titans when he was close to being glorified, he suddenly began to have a double vision followed by a thunder clap in his head which made him misread the lines in the responsorial psalm and from the usual, "yeah baby" he screamed "baby, my head". He clasped his head with his hands in an attempt to prevent it from exploding, he freezed and slumped.

The fun had become a nightmare and Eka, as called by her mother-in-law, felt like she has been given a cup of vinegar to drink. She held her husband, shook him hard, while calling on him "honey! Honey!! Honeybee!!! But he had passed out. He was rushed to the hospital where he was confirmed dead. Agatha accused Ekaite of being responsible for Jeffrey's death but I think otherwise.


SUBARACHNOID HEMORRHAGE

Is a condition when artery (ies) bleed spontaneously into the subarachnoid space( space between the coverings of the brain). The bleeding may arise from an abnormal artery with a balloon (aneurysmal hemorrhage).Factors that can predisposed one to have such condition includes smoking, hypertension, alcohol abuse. Others include connective tissue disorder, polycystic kidney disease and  first degree relative.

It's associated with photophobia (aversion to light), loss of concentration, vomiting, neck stiffness and focal neurological signs like hemiparesis (paralysis of half of the body). It can be triggered by physical exertion, straining and sexual excitement. Jeffrey's death could have been prevented by life style modification and regular medical check up. Anyone who is forty years and above should make it a point of duty to attend regular medical check up.

STONE IN THE BREAST

Rhoda was wheeled into the Accident and Emergency section of the hospital. She suddenly could no longer walk with associated history of severe headache and back pain. She was 33 years old and a mother of four children. Her husband, Mr. Damian, came in the company of his younger brother probably for moral or financial support. Anxiety was written on their faces while confusion dangled over their chests like the necklace of Rick Ross. A young lady with unkept hairdo, a haphazard buttoned shirt and occasionally scratching her hair as though their were termites in it kept calling on Rhoda with such gracious words to sooth her pain and give her hope.
It all started four months ago when Rhoda noticed that her three months old baby would refuse to suckle her right breast when put to it. When she squeezed the same breast, bloody discharges mixed with breastmilk were apparent but she kept this from her husband and sister-in - law for about a month despite feeling a lump in the same breast. Pain which afflicted the breast alongside appreciable growth of the lump broke her silence. Her sister-in-law was of the opinion that the breastmilk from the affected breast had gone sour but for the lump, she suggested a herbal concoction that would dissolve it and possibly remedy the sourness of the breast. Yours faithfully, Mr. Damian, bought the idea, dashed out like one with fecal incontinence with watery diarrhea and procured the drugs which his wife took for two months. Of course if it was effective they wouldn't present to the hospital.
Back in the hospital, I enquired, was she sick before now? Mr. Damian reluctantly responded, my wife has a stone in her breast. The word 'stone' pricked me like the acupuncture needles and my heavy and weary eyes opened as though there was lightening in the room. What do you mean by that? I enquired . Then he doused my curiosity when he added that 'it was a spiritual attack', then why are you here? You should take her to the church, I rebuffed,to bruise his ignorance but he broke down in tears and said, 'Please help us sir'.
When I examined her breast, both appeared like an orange peal, the nipples had retracted and both breasts were harder than Olumo rock. Her health deteriorated so fast, she lost consciousness two days later and never woke up.

IMPORTANCE OF SELF BREAST EXAMINATION AND EARLY PRESENTATION TO THE HOSPITAL

Rhoda had malignant cancer of both breasts which actually started from the right but her delay gave room for it to spread to the left breast, bones and brain. This explains her pain, paralysis and eventual loss of consciousness. Her liver was also enlarged and tender suggestive of possible spread to it.
The lump may be a benign (harmless) or a malignant (can cause death) as in the case of Rhoda. Please note that benign condition may transform into malignant if not attended to.

FIVE YEARS OF PREGNANCY - DEMYSTIFIED


Depression and frustration have become her companions. She is tired of carrying her perceived pregnancy for five years. Her life has been decorated with bitterness and agony of childlessness, worsened by her mother-in-law who made her appreciate hell in her matrimonial home. She thought it was over when she missed her menstrual period for three consecutive months but here visit to the clinic robbed her of her joy although momentarily. The pregnancy test result came out negative and the doctor tried in futility to explain this to her. Angry and disappointed, she tore the result and even accused the doctor of conniving with her mother-in-law to make her life more miserable.
She was convinced that she was pregnant based on the signs and symptoms she noticed ; her abdomen was growing, her breast became engorged, she was nauseating with occasional episodes of vomiting and she could feel the baby moving within her. Encouraged by this, she visited another clinic and as usual left angrily with the words 'The God whom I serve will surprise you all'. She visited the market and bought clothings in preparation for her delivery.
Its now one year and she is still "Pregnant". She visited her prayer team and those who claimed that they could  see beyond accused her mother-in-law of being responsible for her predicament. They further informed her that the child she was bearing is a "Destiny Child" and that God hid him away from the world for a purpose. Adaugo was gingered, she bought every ideology they sold to her including paying a chunk sum of money for special prayers.
Its now five years and Adaugo is still pregnant. She is tired of all forms of Abracadabra and she is back to the clinic.

PSEUDOCYESIS

This is a psychological disorder in which a Non-pregnant woman thinks she is pregnant with signs and symptoms suggestive of pregnancy as seen in the case of Adaugo. It may occur in ages of 7-79 years. Alongside blindness, paralysis, diplopia (double vision), amnesia (loss of memory), psychogenic nonepileptic seizure, dysphagia..... it is a conversion process in which psychic conflict is transformed into physical effect.
The perceived fetal movement is intestinal movement, the loss of menstruation is mostly due to problems of the hypothalamus (could also be ovarian failure) and engorgement of the breast occurs secondary to hyperprolactinemia which may be due to drugs eg Cimetidine, methyldopa, phenothiazine. Other causes may include Pituitary Adenoma( Tumor of the pituitary gland) or prologed nipple stimulation.

ANGRY FATHER'S BLOW SENDS ONLY CHILD TO EARLY GRAVE

The discolouration of her face wasn't the product of a make up artist neither the scarification marks on her body that of tribal marks . The different shapes and healing stages of the scars gives a clue to possible objects of battery and duration. Yet she remained faithful and persevering in her marriage, hoping that one day, the man that once loved and swept her off her feet by his gestures would one day come back to his senses and love her once again but the soluble ego in alcohol encapsulated her wishes/prayers. 
One would think that the birth of Agnes into the family would re-write the script or better still re-assign the character given to Mr. Dimgba which he played so well as to make him a new OSCAR Award nominee.
The third day of August will always come with a scapel to open the wounds in the heart of Mrs. Eunice, a wound deeper than those due to her husband's assault and for which no therapy could stop its recurrent breakdown. This night is darker than all nights with a bag full of regrets, a night of horror, when the angel of death moved over the land but couldn't see the blood of the lamb on Dimgba's door post. He came in drunk as usual, demanding for his supper to be brought but his wife ignored him as no money was made available to her and she was almost running out of her little savings from petty trading. This night she had no appetite to stomach any trouble so she retired to one corner of the room like one dejected with her eleven months old Agnes who she carried in her arms. Yet he would not let the toothless bull dog be. Her silence provoked him into doing what he knows to do best with his wife. Irritated and frustrated by his actions, Mrs. Eunice pushed her husband away but his furious recoil with the intention to punch her face landed on his daughter's head. The trauma was so severe that Agnes began to fit. She was rushed to the hospital by her mother alone with the help of her neighbour, a transporter. Agnes was resuscitated but couldn't be admitted for financial reason but as they were making their way back to the house that night under the rain, their tricycle slipped into the gutter while trying to avoid a pot-hole and Agnes sustained another trauma to the head from the bar of the tricycle, fitted the second time and lost consciousness. She was confirmed dead on arrival to the hospital the second time. The news of her death severed the blood supply to her mother's brain and she slumped. WHO ACTUALLY KILLED MISS AGNES? Was it the Father, mother, government, or cyclist?

THE DEMON CALLED CEASEREAN SECTION-1

To say she is beautiful is an understatement, fair, tall with all the natural endowment that could distract a man from his focus but like everyone she had two notable weaknesses or would I rather say one. Mrs. Ikenga was in her mid-thirties, having been married for about ten (10) years without an issue. She had fasted like Gandhi, prayed like Hannah and visited other places in search of a miracle.
Fate eventually smiled on her at 35years. When she broke the news to her husband, it was like announcing to Buhari that he won the election. Highly elated and in the spirit of celebration he gave the wife a key to a brand new Infinity jeep two days later.

At the later end of her regular antenatal visits, the gynecologist after examination recommended "Ceaserean Delivery" (CS) on the ground that her pelvis was contracted in addition to an ultrasound scan result indicating an abnormal lie (transverse lie) but she quickly rebuked the physician, rejecting the suggestion over her life while tapping her fingers and swinging her hands over her head . She also accused him of attempting to extort her by opting for CS. Dr Cyril didn't see her again . Mrs. Ikenga went to her pastor who told her that there is nothing God cannot do, quoting the scripture, assuring her that she would deliver like the "Hebrew Women". A prayer and fasting program was scheduled but mid way to this, labour started while in the church, she was taken to a maternity centre. After two days of labour, she was rushed to the Teaching hospital, covered in a pool of her blood and unconscious, having ruptured her uterus(womb) . She was quickly wheeled into the theatre and the ultimate price to save her life was to remove her womb so as to stop the bleeding . Of course her long expected child did not make it,she was just lucky to be alive but still had the CS she rejected initially .

DANGERS OF EARLY PREGNANCY - AN 18YR OLD IMPREGNATED BY A 17YR OLD

, Ekanem was Madam Njideka's househelp for about seven (7) years. Her hope of at least obtaining a Secondary School Certificate was first sacrificed on the alter of poverty as her madam could no longer sponsor her education just when she was a year away from graduation, SS2 ( Senior Secondary, Class 2). She was now a full time businesswoman, assisting her madam in buying and selling of food stuff in one of the popular markets in Aba, Nigeria.
It was at this market that she met with Daniel, a 17 yr old SS3 student who coincidentally lives in the same street about two hundred metres apart.. Juvenile delinquency wrapped them up in an unholy romance. Daniel was eager to assist his insensitive parents with sales at their shop adjacent to Ekanem's kiosk and would volunteer to stay on and pack up the goods while his parents left for the house, a role he had hitherto refused with utter bitterness and complaints mixed with murmuring as though he was enslaved.
The two love birds would go home together, divert into an uncompleted building along their part and explore the rainforest. This happened for about three months with neither of the families finding out. In the fourth month of their sex escapade, Ekanem became unwell with symptoms of loss of menstruation, tiredness, nausea and vomiting, salivating and lower abdominal pain. When the laboratory result was handed over to Mrs Njideka, her eyes were dilated with surprise and fury with attempt to disfigure Ekanem's face, pounding her with her oversized Christian mother's arms while making enquiry about who was responsible. The slapping that gave her a glimpse of stars made the truth run out of her mouth like hurricane Maria and the thought that it was a teenager made her crazy. She formed a fist, trapped Ekanem's head between her massive thighs while drumming out the devil on her back. It took a basket of plea to rescue Ekanem from the lion's den.
Mr and Mrs Osondu, Daniel's parents, accepted responsibility for the pregnancy and relocated their son to Lagos for safety after threats to his life. She was registered for Antenatal care. Just when all seemed to be well, the DREADED DANGER OF EARLY PREGNANCY ; PREECLAMPSIA AND ECLAMPSIA (Hypertensive disease in pregnancy with convulsions) came at the 9th month. She had four episodes of convulsion with Severe hypertension, was brought to the hospital and eventually delivered of a baby boy through Ceaserean Section (C/S). Ekanem and the baby are doing well but although yesterday she was reported to have collapsed when she visited the rest room and was resuscitated. It was due to poor nutrition.
Apart from PREECLAMPSIA, ECLAMPSIA, INCREASE RISK OF CEASEREAN SECTION AND MATERNAL MALNUTRITION, INTRAUTERINE GROWTH RESTRICTION, PSYCHOSOCIAL PROBLEM, POSTPARTUM HEMORRHAGE AND MATERNAL/FETAL DEMISE amongst others are the DANGERS OF EARLY CONCEPTION (ADOLESCENT).

PASTOR, ASSISTANT AND MEMBERS PLACED BEHIND BARS OVER ILLEGAL DETENTION OF NEIGHBOUR


It was midnight but the peace and serenity that characterize this hour was for Mr Akpantu, a 63yr old petty trader/labourer, stolen by Pastor Elijah and members of his congregation who had convened for a vigil within the neighbourhood. He tried hard to ignore the theatre of spiritual warfare, the weeping and echoes of fire which the church relied heavily on as a weapon to destroy their passived enemies. Suddenly, his threshold for tolerance had been bruised and like an agitated python danced his way to the church to end the perceived ugly trend which had become almost a daily affair.
He banged at the gate in a proportion equal to the noise and despite the cracking pain he felt he was more intense with vigor but all to no avail. Infuriated by his failure to catch the attention of members of the church who as at this time were engrossed in their 'kabashing', he decided to take a more drastic measure. Looking around, he found pieces of rocks littered like the twelve tribes of Israel. His first throw did the magic. At this the pastor seeing him mobilised the praying members who went after Mr. Akpantu, apprehended him, bundled him into the church and had him bound in ropes while they conducted deliverance on him.
The following morning he was rushed to the hospital where he spent two weeks. This opened another chapter to the story. His relatives reported the incidence to the Nigerian Police, prompting the arrest and detention of the pastor, his wife and some members of his church who were present that night for the vigil.

A 15YEAR OLD ABDUCTED AND RAPED FOR 14DAYS

This  is the day when a burning arrow dipped in poison pierced the heart of an unsuspecting victim and left her devastated, a day when the heavens stood still in awe of man's inhumanity to man, a day when the garment of a virgin was indelibly stained with blood.
It was Feb 10, 2017, Tochi Chukwuka, a fifteen year old SS2 student in her routine activity left for school in the morning . The conclusion of the day's academic activities gave way to the misfortune that befell her. A young man had stalked her from her school gate to her bus stop using a tricycle popularly know as "Keke". After alighting, he went after her as she made her way home. After few steps away from the bus stop, he caught up with her and immediately began making advances at her amidst passersby and bystanders. He touched her arm repeatedly and that was all she could remember.
The next time she opened her eyes, though her vision was blurry, she noticed she was in bed in a strange room, an excruciating pain between her thighs drew her attention to her private part. The probing finger was stained with blood. She was still in a state of confusion when the rapist returned. His dreadlocks, unkept beard, furrowed face, well developed muscles, discrete tattoos which featured a scorpion and cross, and stench of alcohol oozing from his sweaty body sent fear through her spine. She pleaded with him to set her free but her plea was thrown into the bin, her second request was to go outside to pee but he offered her a bucket instead, her objection earned her a slap, another round of rape in which she was forced to smoke cigarette and she lost consciousness again.
Back home the worried parents had visited the school, police station, printed and circulated her picture. A tip off from a concerned individual led the police to the accommodation of the rapist, he was nabbed at about 2am on Feb 24 2017 and it was evident that at that unholy hour, he was busy raping his victim. This happened in the industrious city of Aba, Abia ,State, Nigeria.

The name and picture are not true identity of the victim but the story is true.
